Bleeding Like a Period While Pregnant

Some causes are minor, but bleeding can also occur with pregnancy loss, an ectopic pregnancy, infection, or a problem involving the cervix or placenta. The amount and colour of blood alone cannot identify the cause.

Light spotting may follow sex, a pelvic examination, or irritation of the cervix because cervical tissue can bleed more easily during pregnancy. Bleeding may also accompany a miscarriage. An ectopic pregnancy can present with vaginal bleeding and pain, often on one side, and can become dangerous if internal bleeding develops. Later in pregnancy, bleeding may relate to the placenta or the start of labour.

A clinician may consider how far along the pregnancy is, whether the bleeding is getting heavier, and whether there are cramps, pelvic pain, fever, dizziness, fluid leakage, or tissue passing from the vagina.

Contact your maternity team, obstetrician, or an urgent care service promptly, even if the bleeding has stopped. Use a sanitary pad rather than a tampon so you can observe the flow, colour, clots, or tissue. Note when it began and any pain or dizziness. Avoid sex until a clinician has advised whether it is safe to resume.

Assessment may include questions about symptoms, an examination, an ultrasound, and blood tests. If you know that your blood group is Rh-negative, mention this because the clinician may consider whether anti-D treatment is appropriate.

Go to an emergency department or call local emergency services if bleeding is heavy or rapidly increasing, or if it occurs with severe abdominal or pelvic pain, shoulder-tip pain, fainting, marked dizziness, weakness, confusion, breathing difficulty, or pale and clammy skin. These signs can indicate significant blood loss or internal bleeding.

Urgent assessment is also needed for fever, worsening pain, fluid leaking from the vagina, regular painful tightenings, or reduced fetal movement after movements have become established. Do not drive yourself if you feel faint or unwell.

Ask what the examination or scan shows, whether repeat blood tests or ultrasound are needed, and which changes in bleeding or pain should bring you back immediately. You can also ask whether your blood group affects treatment, whether you should change activity or sexual activity, and who to contact if symptoms return outside clinic hours.
