Black toenail falling off: safe care and warning signs

This often follows a stubbed toe, a dropped object or repeated pressure inside footwear. Separation can also accompany infection or another nail condition, so unexplained colour or loosening deserves assessment rather than being assumed to be a bruise.

Trauma can disrupt the bond between the nail plate and the skin beneath it. The dark colour is commonly dried blood, while a new nail may begin forming underneath. The old plate can remain partly attached for a while and may catch on socks or shoes.

Yellowing, crumbling, odour or debris can point towards fungal involvement. Greenish colour, pus, increasing tenderness or hot swollen skin suggests infection around the nail. Dark pigment arising without injury, particularly if it extends onto nearby skin or changes rather than growing out, should be evaluated by a dermatologist.

Wash gently with soap and water, pat the toe dry, and cover any exposed tender area with a clean non-stick dressing. Choose roomy shoes or sandals that do not press on the nail. If an entirely loose edge is catching, a clean clipper can trim only that unattached portion; leave firmly connected nail in place.

Do not tear the plate away or push tools underneath it. If there is a large painful collection of fresh blood, a clinician can decide whether pressure relief is suitable and check for a deeper injury. Keep watching the emerging nail and the skin around it for new colour, drainage or swelling.

Obtain urgent assessment if the toe is misshapen, numb, very painful, difficult to move or still bleeding after firm pressure. Prompt care is also appropriate for spreading redness, warmth, pus, fever or red lines tracking up the foot.

Contact a clinician soon when the nail detaches without an obvious injury, pigment spreads to the cuticle or surrounding skin, or the new nail grows with persistent dark colour or distortion. If you have diabetes, poor circulation, reduced sensation or immune suppression, seek guidance early and avoid trimming a detached nail yourself.

Explain when the colour appeared, any injury or shoe pressure, and whether discomfort is improving. Ask if the toe needs imaging, drainage, infection testing or examination of the nail root. It is also useful to clarify how to dress the area and when normal footwear or activity is reasonable.
