Why the appearance matters

The first stools after birth are often dark, thick, and sticky; this early stool is called meconium. A later new black stool, especially one that looks like tar, may contain digested blood and needs assessment. Iron-containing feeds or supplements can also darken stool. Gastritis, Helicobacter Pylori Infection, and peptic ulcer are medical terms a clinician may consider in digestive symptoms, but caregivers should not diagnose these conditions from colour alone. The infant’s age, feeding, alertness, and other symptoms guide urgency.

What caregivers can do

Contact the infant’s paediatric clinician promptly and explain whether the stool was an expected early stool or a new change. Take a clear photo, keep the nappy if practical, and note feeding, wet nappies, vomiting, fever, and medicines or supplements. Do not give an infant pain medicine, herbal remedy, or stomach treatment unless prescribed. Continue usual feeds if the infant is alert and feeding normally, but do not delay care if feeding becomes difficult.

Get urgent help

Seek emergency care for an infant who is difficult to wake, floppy, pale or blue, breathing abnormally, vomiting blood or green fluid, refusing feeds, repeatedly passing black tarry stool, or producing clearly fewer wet nappies. A swollen belly, persistent crying with illness, fever in a very young infant, or any rapid deterioration also warrants immediate evaluation. Do not wait for another nappy to confirm the change.

Ask whether the stool fits normal meconium, whether a medicine or supplement could explain the colour, and whether examination or testing is needed. Tell the clinician when the stool appeared, its texture, the infant’s feeding pattern, and any vomiting, fever, pallor, sleepiness, or change in wet nappies.
