What black, hard stool may mean

Stool may look dark after iron tablets, medicines containing bismuth, or strongly coloured foods. Constipation can make stool dry, firm, and difficult to pass, but hardness does not explain every change in colour.

Melena is stool made dark by digested blood, commonly from bleeding in the oesophagus, stomach, or first part of the small intestine. It is often jet black, sticky or tar-like, and unusually strong-smelling. Gastritis, Helicobacter pylori infection, and a peptic ulcer can present with this type of stool, sometimes alongside upper-abdominal pain, nausea, weakness, or dizziness.

A single dark stool after a known medicine or food may have a harmless explanation, yet colour alone cannot confirm the cause. A clinician may ask about medicines, examine you, and arrange stool or blood tests and further evaluation when bleeding is a concern.

What to do now

Notice whether the stool is genuinely black and tarry or simply dark brown. Record when it began, how often it has happened, and any recent iron, bismuth, food, pain-relief medicine, or supplement use. Do not stop a prescribed medicine without speaking with the prescriber.

Drink fluids if you have no medical fluid restriction, and increase fibre gradually for hard stools. Gentle movement and a regular toilet routine may help constipation. These measures are not a substitute for assessment when stool is tarry, recurrent, or accompanied by other symptoms.

Seek urgent medical care if black tarry stool occurs with vomiting blood or material resembling coffee grounds, fainting, marked dizziness, confusion, severe weakness, shortness of breath, or severe abdominal pain. These can accompany significant digestive-tract bleeding.

Arrange prompt medical advice for repeated black stools, unexplained black stool, or black stool while taking medicines that can irritate the stomach or affect bleeding. Contact a clinician sooner if you are pregnant, have a history of ulcers or digestive bleeding, or cannot keep fluids down.
