Black and hard stool: what it may mean and what to do

Short answer
Black and hard stool may come from iron or dark foods, but tar-like black stool can indicate bleeding and needs medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible harmless causes
- Iron, bismuth medicines, or dark foods
- Texture to watch
- Black, tarry, sticky, or unusually offensive stool
- Priority
- Urgent advice if unexplained or accompanied by weakness or dizziness
What black and hard stool can mean
Iron supplements, bismuth-containing stomach medicines, and deeply coloured foods can darken bowel movements. Constipation can make stool firm, difficult to pass, or pellet-like. The appearance is more concerning when the stool is truly black, tarry, unusually sticky, or has a strong offensive smell. Blood digested in the stomach or small intestine can turn black before it leaves the body. Gastritis, Helicobacter Pylori Infection, and Peptic Ulcer are conditions that can affect the upper digestive tract and may be associated with this pattern.
Colour alone cannot identify the cause. Tell a clinician about abdominal discomfort, vomiting, dizziness, weakness, recent medicines, and any change from your usual bowel habit.
What to do now
If the stool became dark after a new supplement, medicine, or food, note the timing and whether the colour returns to normal. Do not stop a prescribed medicine without advice. For hard stool, drink fluids if you have not been told to restrict them, add fibre gradually, and move gently as tolerated. Avoid straining. Arrange medical advice promptly if the black colour is unexplained, repeats, or occurs with upper-abdominal pain. A clinician may review medicines and decide whether examination or testing is needed.
When to seek urgent care
Seek urgent medical help for black tarry stool with faintness, marked weakness, confusion, shortness of breath, a racing heartbeat, severe abdominal pain, vomiting blood, or vomit that looks like coffee grounds. Repeated black bowel movements also deserve prompt assessment, even if constipation is present. These signs can accompany digestive bleeding or loss of fluid and should not be managed only with home constipation remedies.
Questions for your doctor
Ask whether your medicines or supplements can explain the colour, whether constipation needs treatment, and which symptoms would suggest bleeding. Mention when the change began, how often it has happened, the stool texture, and any pain or dizziness. You can also ask whether Gastritis, Helicobacter Pylori Infection, or Peptic Ulcer should be considered.
