How to take bisphosphonates safely

For an oral dose, the usual approach is to swallow the tablet whole with plain water on an empty stomach, remain upright, and wait for the period stated on the label before eating, drinking anything else, or taking other medicines. An intravenous dose is administered by a clinical team, which will check that treatment is appropriate for you.

Bisphosphonates slow the cells that break down bone. In cancer care, they may be used to protect bones weakened by disease, reduce problems caused by bone damage, or manage a high calcium level related to cancer. The schedule varies by medicine, treatment goal, kidney function, and whether the drug is oral or intravenous.

These medicines can irritate the food pipe when taken by mouth. They can also lower blood calcium and, rarely, affect the jaw bone or contribute to an unusual thigh-bone injury. Your team may arrange blood tests and ask about dental health before or during treatment.

Follow the medicine label rather than a generic schedule. For tablets, use a full glass of plain water; mineral water, tea, coffee, juice, food, antacids, calcium, and iron can interfere with absorption. Do not chew or suck the tablet, and do not lie down during the waiting period specified for your medicine. If you miss a dose, check the written instructions or contact the prescribing team instead of doubling it.

Before an infusion, tell the team about kidney problems, low calcium, pregnancy, planned dental work, or new mouth symptoms. Keep well hydrated if your clinicians have said this is safe for you. Maintain dental hygiene, attend recommended dental checks, and tell your dentist that you receive a bisphosphonate before an extraction or implant is planned.

Get urgent help for trouble breathing, swelling of the face or throat, fainting, or another sign of a severe allergic reaction. Contact your cancer team promptly for painful or difficult swallowing, severe chest pain after a tablet, persistent vomiting, muscle spasms or tingling around the mouth, a mouth sore that does not heal, exposed jaw bone, or new jaw pain.

Report new pain in the thigh, hip, or groin, especially if it persists or occurs without a clear injury. After an infusion, fever, aches, or fatigue can occur; ask the treatment team which symptoms are expected and which should trigger an urgent call, particularly if you are also receiving treatment that affects immunity.

Ask for the exact medicine name, dose schedule, tablet waiting period, and instructions for a missed dose. Also ask whether you need calcium or vitamin D, blood tests, kidney monitoring, or a dental assessment; whether any current medicine should be separated from the tablet; and whom to call if side effects develop outside clinic hours.
