Birth control and bleeding between periods

Spotting is also called breakthrough bleeding. It may settle as the body adjusts to a hormonal method, yet the same symptom can occur with missed doses, pregnancy, infection, or a condition affecting the cervix or womb. Its pattern and associated symptoms matter.

Hormonal contraception changes the womb lining. A thin or unstable lining can shed at unexpected times, particularly after a new method, a switch in dose, inconsistent pill-taking, or delayed replacement of a method. Vomiting, diarrhoea, and interacting medicines can also matter for some contraceptives.

Chlamydia can present with bleeding between periods or after sex, often alongside discharge or pelvic discomfort, though it may cause no other symptoms. Cervical dysplasia, cervical cancer, womb cancer, endometrial polyps, and uterine fibroids can also present with abnormal bleeding. These possibilities are reasons for appropriate assessment, not conclusions that can be drawn from spotting alone.

Use a diary to record spotting, normal periods, flow, clots, pain, bleeding after sex, discharge, and contraceptive doses. Check the instructions supplied with your method after any late or missed dose. If pregnancy could have occurred, follow advice on pregnancy testing and whether backup or emergency contraception is needed.

Schedule a clinical review when bleeding keeps returning, starts after a stable period on the method, or affects daily activities. The clinician may discuss a pregnancy test, infection testing, cervical screening status, examination, blood tests, or ultrasound according to your history. Do not discontinue contraception without another pregnancy-prevention plan if avoiding pregnancy matters to you.

Seek urgent care if bleeding is heavy and you feel faint, very weak, short of breath, confused, or severely unwell. Sudden or intense pelvic pain, shoulder-tip pain, collapse, or bleeding with a positive pregnancy test also warrants urgent assessment for a pregnancy complication or internal bleeding.

Request a prompt appointment for bleeding after sex, foul-smelling or unusual discharge, fever, pelvic pain, or a new pattern that does not settle. Keep cervical screening up to date when invited, but remember that screening is not a diagnostic test for current symptoms; unexplained bleeding still needs its own assessment.

Ask whether your exact method commonly causes this pattern, whether contraceptive protection may be affected, and which tests fit your symptoms. Clarify what changes would require urgent help, whether a different dose or method could improve bleeding control, and how to switch without leaving a gap in pregnancy prevention.
