Bilateral angular cheilitis: soreness at both mouth corners

Saliva pooling, repeated lip licking, dry or irritated skin and the shape of the mouth corners can weaken this area. Yeast or bacteria may then contribute. Treatment depends on the cause, so recurring or stubborn soreness is better examined than repeatedly covered with assorted creams.

When both angles stay moist, the skin softens and splits during speaking, eating or yawning. Redness, crust, shallow fissures, burning and tenderness can follow. Dentures that alter lip support, drooling during sleep or frequent wiping may keep the cycle going.

A clinician may consider contact irritation from dental or lip products, oral thrush, eczema, infection, denture fit and health or nutritional factors when the condition recurs. Bilateral involvement describes location, not a single cause. Changes elsewhere on the lips or inside the mouth can help narrow the diagnosis.

Gently clean away food and saliva, pat the corners dry and apply a bland fragrance-free barrier emollient to reduce wetting and friction. Avoid licking, picking crusts and using strongly flavoured lip products. Keep dentures clean and remove them as instructed by your dental professional.

Do not share lip products. Avoid using leftover antibiotic, antifungal or steroid creams without a diagnosis: each treats a different process, and a topical corticosteroid used alone can obscure or worsen some infections. Prescribed treatment should be applied only for the advised period and location.

Seek a medical or dental review if cracks keep returning, fail to heal, bleed repeatedly, spread onto nearby skin, or occur with white patches inside the mouth. Increasing pain, pus, honey-coloured crust, warmth or swelling may indicate infection and should be assessed promptly.

Urgent help is needed if lip or tongue swelling affects breathing or swallowing. A persistent firm area, unexplained ulcer or significant difficulty eating also warrants timely assessment rather than continued self-treatment.

Ask whether yeast, bacteria, irritation, denture fit or another skin condition is contributing, and whether a swab or health assessment is appropriate. Confirm which barrier or medicated product belongs on the corners, how to apply it, and what changes would signal that the diagnosis or treatment needs reconsideration.
