Beginning cellulitis: early signs and next steps

It is a bacterial infection in deeper skin tissue, commonly starting where skin is cracked, cut, ulcerated, or affected by a fungal foot infection. A clinician may need to prescribe an antibiotic, so do not rely on skin creams or home remedies alone.

Cellulitis commonly presents with redness, warmth, swelling, and pain on one area of skin. The border may be poorly defined rather than sharply outlined, and the affected area can enlarge over hours or days. Fever, chills, or feeling unwell may develop as the infection progresses.

Similar changes can have other causes, including irritation, an insect reaction, a blood clot, or inflammation without infection. An examination helps distinguish them. Darker skin may show less obvious redness, so compare warmth, tenderness, swelling, and colour with the surrounding skin.

Arrange a same-day medical assessment when cellulitis is suspected. Keep the area clean, rest the affected limb, and elevate it when practical to ease swelling. If a clinician starts antibiotics, take them exactly as directed and complete the prescribed course unless they advise otherwise.

You can mark the edge of redness with a pen and note the time to help track spread, but do not delay care while monitoring. Avoid squeezing, puncturing, or vigorously massaging the area. Mention broken skin, athlete's foot, diabetes, poor circulation, recent surgery, or immune-suppressing medicines during the assessment.

Seek urgent medical help if redness spreads rapidly, pain becomes severe, the skin blisters or turns dusky, or you develop fever, confusion, dizziness, fast breathing, marked weakness, or difficulty staying awake. These features can signal a deep or spreading infection.

Prompt assessment is also warranted when the affected skin is near an eye, when you have reduced immunity or poorly controlled diabetes, or when symptoms worsen after treatment begins. Call emergency services for collapse, severe breathing difficulty, confusion, or other signs of critical illness.

Ask what features support cellulitis, whether any alternative diagnosis is possible, and how soon improvement should become noticeable. Clarify how to take the medicine, what to do after a missed dose, which pain relief suits you, and which change should trigger reassessment.
