Bad shin splints: how to respond and recover

Short answer
Bad shin splints cause exercise-related pain along the inner border of the shin and should prompt a reduction in impact loading, not continued training through pain.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Exercise-related tenderness spread along the inner shin
- Recovery focus
- Reduce painful impact, restore capacity, then progress loading gradually
- Needs review
- Pinpoint bone pain, night pain, limping, or symptoms at rest
The short answer
The discomfort often develops after the lower leg has been exposed to more running, jumping, hills, or hard surfaces than it can currently tolerate. Recovery depends on calming symptoms and rebuilding load gradually.
How shin splints behave
Shin splints, also called medial tibial stress syndrome, typically present with tenderness spread along a length of the inner shin rather than one isolated point. Pain may appear at the start of exercise, ease as the body warms up, then return afterward. As irritation increases, ordinary walking can become uncomfortable.
Training errors are common contributors, while foot mechanics, calf capacity, footwear, and running technique can affect how load reaches the tibia. Flat feet may alter lower-limb loading in some people, but foot shape alone does not establish the cause. A focused examination can separate shin splints from a bone stress injury or other sources of shin pain.
Settle pain and rebuild load
Temporarily replace painful impact exercise with an activity that does not reproduce symptoms. Review recent changes in distance, pace, hills, surfaces, and footwear. When everyday walking is comfortable, reintroduce running in manageable bouts and avoid increasing several training demands at once.
Physiotherapy can address calf strength, ankle mobility, balance, and load progression. Gait retraining may be considered when running mechanics add excessive stress, while custom orthotics may suit selected people after assessment. Neither equipment nor technique changes replace a graded return to activity.
When symptoms need assessment
Arrange an examination if pain becomes sharply localised, occurs at rest or at night, causes a limp, or persists despite reducing impact. Those features can accompany a bone stress injury and should not be managed by repeatedly testing the leg with another run.
Seek urgent care if severe lower-leg pain and tight swelling develop rapidly, particularly with numbness, weakness, or difficulty moving the foot. A cold, pale foot or major traumatic injury also requires emergency evaluation.
Planning with your clinician
Ask what findings support shin splints, whether the pain pattern raises concern for bone injury, and which pain-free exercises fit your current stage. A useful return plan should define how to progress impact, what response is acceptable afterward, and which symptoms mean you should step back.

Questions people ask
Can I keep running with shin splints?
Running through escalating pain can prolong irritation. Use pain-free alternatives until walking settles, then follow a gradual return that does not cause a clear flare during or after exercise.
Are shin splints the same as a stress fracture?
No. Shin splints usually present with broader tenderness along the inner shin, while a stress fracture commonly presents with more focal bone pain. Examination and, when appropriate, imaging help distinguish them.
Do orthotics cure shin splints?
Orthotics may help selected people manage lower-limb loading, but they are only one possible part of care. Training adjustments, strength, mobility, and a graded return remain central.
