Why spasms happen

A spasm can follow an awkward lift, sudden turn, prolonged sitting, or muscle strain. It may also occur with facet joint syndrome, a herniated disc, or non-specific low back pain. The tightness can make standing or changing position difficult, but the spasm itself does not reveal which structure is involved. Pain travelling into a leg, tingling, numbness, or weakness gives a clinician more useful information than intensity alone.

Ways to ease the episode

For the first day, reduce provoking movements without staying completely still. Take short, easy walks and change position regularly. Try warmth if it relaxes the area, or a wrapped cold pack if that feels more comfortable. Avoid forceful stretching, heavy lifting, and repeated bending while movement is sharply painful. If episodes recur, physiotherapy may address movement habits, trunk strength, and muscle control; techniques such as myofascial release or dry needling may be considered after assessment.

Warning signs

Get urgent help for new leg weakness, numbness around the genitals or anus, loss of bladder or bowel control, fever with back pain, major trauma, or pain that is severe and rapidly worsening. Arrange a clinical review if the spasm repeatedly returns, prevents sleep or work, or is accompanied by persistent leg pain, pins and needles, or unexplained weight loss.
