Back steroid injections: purpose, procedure and aftercare

It is not a general cure for every type of back pain. The proposed target, expected benefit and risks should match your symptoms, examination and any relevant imaging.

The injection usually combines a corticosteroid, which reduces inflammatory activity, with or without local anaesthetic. Depending on the suspected source, medicine may be placed near a spinal nerve, in the epidural space or around a small spinal joint.

Image guidance may be used to confirm needle position. Temporary numbness or heaviness can come from local anaesthetic, while the steroid effect may take longer to judge. Relief can vary, and a good response does not by itself prove a single diagnosis.

Give the treating team a full medicine list and disclose blood-thinning treatment, diabetes, allergies, infection symptoms, pregnancy possibility and previous reactions to injections or contrast material. Follow their instructions about eating, medicines, transport and activity because these depend on the planned procedure.

Afterward, protect a temporarily numb or weak limb and follow the discharge plan. Keep the injection site clean, note changes in pain and function, and avoid using short-term relief to rush back into heavy activity. The longer aim is often to make rehabilitation and normal movement more manageable.

Short-lived soreness, flushing or a temporary rise in blood glucose can occur. The clinician should discuss risks relevant to the injection route, including bleeding, infection, headache, nerve injury and steroid-related effects.

Seek urgent medical help for new or worsening limb weakness, loss of bladder or bowel control, numbness around the groin, severe breathlessness, facial swelling, or a rapidly developing allergic reaction. Contact the treating service promptly for fever, increasing redness or drainage at the site, a severe persistent headache, or pain that escalates unexpectedly.

Ask which structure will be targeted, why your clinical pattern supports that choice, whether image guidance and contrast are planned, what alternatives exist, and how the result will change the next stage of care. Also clarify which medicines require special instructions and whom to contact after discharge.
