What a lower back spasm is
It can cause a sharp grab, hard knot or difficulty straightening after bending. Its intensity does not reliably show how much tissue has been injured, and many mechanical episodes fluctuate with movement.
What may be behind the tightening
Lifting while twisted, an abrupt change in training, prolonged sitting or an unfamiliar task may overload lumbar muscles. The body may also tighten these muscles in response to irritation from spinal joints or discs.
Muscle strain commonly presents with local pain and spasm. Non-specific low back pain describes symptoms that cannot be assigned to one precise structure after assessment. A herniated disc can present with back pain and guarding, particularly when a nerve is irritated, but a spasm alone does not show that a disc is involved.
What to do during the first few days
Find a tolerable position, slow your breathing and avoid repeatedly testing the painful movement. As the grip releases, alternate short periods of walking with rest. Use your hips and legs when rising, and keep loads close to the body.
Complete bed rest can increase stiffness, so maintain ordinary activity within manageable limits. Heat can help some people relax the area. Resume bending and lifting progressively rather than waiting for every sensation to disappear. Assessment is useful if you cannot find a comfortable movement or episodes keep returning.
When lower back spasms require medical attention
Get urgent help for new bladder or bowel control problems, numbness in the saddle area, rapidly worsening leg weakness or an inability to walk safely. Severe pain after a substantial impact also warrants prompt assessment.
Contact a clinician promptly if pain accompanies fever, unexplained illness, abdominal pain or urinary symptoms. Arrange a non-emergency review when pain radiates down a leg with persistent numbness, regularly returns, or continues to restrict sleep, work or mobility.

Should I stretch a lower back spasm immediately?
Do not force a strong stretch into sharp pain. Start with a comfortable position, calm breathing and small movements, then increase motion as guarding eases.
Does a severe spasm mean a serious back injury?
Not necessarily. Muscles can contract powerfully in response to irritation, and symptom intensity alone does not identify the cause. Neurological changes, trauma and systemic illness are more important warning signs.
Is walking suitable with a lower back spasm?
Short, easy walks are often tolerable once the initial contraction settles. Reduce the distance or stop if walking causes escalating pain, new weakness or spreading numbness.
