The short answer

As the abdomen grows, the trunk and hip muscles adapt to a different centre of mass. Ligament changes, fatigue, prolonged positions, and unfamiliar lifting can add to the load. A gripping muscle sensation is not enough by itself to identify the cause.

Why pregnancy changes the back

The abdominal wall and back muscles share the work of supporting the trunk. Pregnancy changes that balance and can make the lower back, buttock, or muscles beside the spine fatigue sooner. A wrapped warm pack on a low setting may soothe muscle, but avoid overheating and protect the skin.

Check with your obstetric clinician or pharmacist before taking any medicine, including products bought without a prescription. A physiotherapist experienced in pregnancy can assess posture, hips, pelvic support, and movement, then adapt exercises as pregnancy progresses.

When to contact maternity care

Contact your maternity unit promptly for back pain with vaginal bleeding, fluid leakage, fever, burning when urinating, abdominal cramping, pelvic pressure, regular tightening, severe headache, visual disturbance, or sudden swelling. Follow the unit's urgent instructions, especially if fetal movement has reduced from your baby's usual pattern.

Seek emergency help for severe sudden pain, collapse, breathing difficulty, chest pain, new weakness in both legs, numbness around the genitals or anus, or loss of bladder or bowel control. After a fall or blow, tell maternity care how you landed and whether the abdomen was struck.
