During a mild episode, avoid the specific lift or twist that triggers the catch, but continue short walks and easy daily movement as tolerated. Try a comfortable supported position and gentle heat for stiffness. A cold pack wrapped in cloth may be preferable after a fresh strain with local tenderness.

Resume loading in small increments once ordinary bending and walking settle. Repeated one-sided episodes deserve an assessment of hip and spine movement, lifting technique, work demands, and muscle capacity rather than repeated rest alone.

Obtain emergency help for new loss of bladder or bowel control, numbness around the groin or saddle area, rapidly worsening leg weakness, collapse, or severe symptoms after a major injury. Fever with flank pain, vomiting, visible blood in urine, or feeling acutely unwell also needs prompt medical attention.

Schedule a non-emergency appointment if episodes become more frequent, wake you from sleep, spread into a limb, or prevent a gradual return to normal activity.
