Azithromycin for impetigo: practical guidance

A clinician selects treatment according to how many areas are affected, whether lesions are spreading, the likely bacteria, allergy history and local antibiotic resistance. Azithromycin should only be used when prescribed for the current infection.

Impetigo is a contagious bacterial skin infection that often presents with fragile sores or blisters followed by crusting. A small, localised area may be managed with a prescribed treatment applied to the skin, while widespread or progressing infection may require an oral antibiotic.

Azithromycin works only against susceptible bacteria. It does not treat eczema itself, fungal rashes or viral skin infections that can resemble impetigo. If the appearance is unclear, recurrent or not responding, a clinician may reconsider the diagnosis or take a sample from a lesion to guide therapy.

Follow the dose and course on the prescription, and do not share the medicine. Mention antibiotic allergies, liver disease, heart-rhythm problems, pregnancy and other medicines before starting. If a dose is missed, follow the label or ask a pharmacist; taking extra can increase side effects.

Gently wash affected skin, pat it dry and avoid picking crusts. Keep fingernails short, wash hands after touching the area, and do not share towels, razors, clothing or bedding. Use any prescribed topical product exactly where directed. Check local school, nursery or workplace advice about returning while the infection is being treated.

Arrange prompt review if sores continue to spread, become increasingly painful, produce marked swelling, involve the eye area, or are accompanied by fever or general illness. A newborn, a person with weakened immunity, or anyone with rapidly worsening skin changes needs timely clinical assessment.

Seek emergency help after azithromycin for difficulty breathing, swelling of the lips, tongue or throat, collapse, or a severe blistering rash. Severe persistent diarrhoea, yellow skin or eyes, or a new irregular heartbeat should be reported promptly to a clinician.

Ask whether the pattern is definitely impetigo, whether a topical medicine would be enough, why an oral antibiotic is needed, and how you will judge improvement. If episodes recur, ask whether an underlying skin condition or repeated scratching is weakening the skin barrier.
