Autism and hearing tests: what families should know

Clinicians may recommend hearing assessment when a child or adult has delayed speech, responds inconsistently to sound, or is undergoing an autism evaluation. Hearing differences and autism can occur separately or together, so each needs its own assessment.

The audiologist may check the ear canal and eardrum, measure how the middle ear responds, and assess reactions to sounds of different pitches and volumes. The approach depends on age, communication style, sensory needs, and ability to follow instructions.

Some methods rely on a person signalling when a sound is heard. Others observe a child's response during play or measure the ear's or hearing pathway's response without requiring a spoken answer. A single incomplete session does not automatically indicate hearing loss.

Tell the clinic about sound sensitivity, preferred communication, anxiety triggers, and any supports that help. Ask whether you can see pictures of the room or equipment beforehand, bring a familiar comfort item, or arrange breaks.

Share any history of ear infections, ear discharge, ear surgery, hearing concerns, and responses to everyday sounds. Avoid coaching a child to give a particular response; the audiologist needs to observe their natural, consistent reactions.

Arrange an assessment if hearing or communication has changed, speech development is a concern, sounds are repeatedly missed, or listening seems much harder in noisy places. Ear pain, discharge, fever, or a blocked sensation also warrants medical advice.

Seek urgent medical assessment for sudden hearing loss in one or both ears, especially with severe dizziness, new weakness, facial drooping, or a severe headache. Urgency is based on the sudden change and accompanying warning signs, not on whether the person is autistic.

Ask which test methods suit the person's communication and sensory profile, what each result means, and whether another session is needed if responses are unclear. You can also ask how hearing findings will be shared with the autism assessment team and what practical support may help at home or school.
