Atrophic scars on the forehead: assessment and care

It may follow acne, chickenpox, a cut, or another local injury. Forehead movement and overhead lighting can make the indentation more obvious. A dermatologist assesses the shape, depth, mobility, colour, and relationship to nearby muscles before suggesting treatment.

The forehead has curved surfaces and moves when you raise or draw together your eyebrows. A depression may deepen, flatten, or shift with expression, giving clues about whether the scar is attached to deeper tissue. The clinician also considers skin thickness and nearby vessels and nerves when choosing a procedure.

A dip in the skin is different from a flat dark mark left by inflammation. Some people have both contour loss and colour change in the same place. Treating only pigment will not restore lost support, while a contour treatment may leave the colour unchanged.

Keep active acne controlled and avoid picking forehead spots. Gentle cleansing and sun protection can reduce irritation and unwanted contrast around a mature scar, but topical skin care cannot release a deep attachment.

Depending on examination findings, microneedling or laser treatment may be considered for texture and remodelling. Subcision may suit a tethered depression, while a dermal filler may support a carefully selected hollow. Forehead anatomy makes precise placement important, and the benefits, limitations, downtime, pigment response, and uncommon complications should be discussed before any procedure.

Have the area examined if it is new without a remembered injury, changing shape, bleeding, repeatedly crusting, painful, numb, or affecting forehead movement. Following treatment or a recent wound, get prompt advice for worsening redness, heat, swelling, discharge, severe pain, eye-area swelling, vision symptoms, or feeling unwell. A stable indentation without these features can usually be discussed at a routine appointment.

Ask how facial movement affects the scar and whether tethering, volume loss, pigment, or a mixture explains its appearance. Discuss which anatomical structures sit near the proposed treatment area, how the clinician adapts the technique for your skin tone, what aftercare is required, and what visible change would count as a worthwhile result for you.
