ASSR hearing test: what to expect

ASSR stands for auditory steady-state response. It is often useful for babies, young children and people who cannot reliably press a button or raise a hand during a standard behavioural hearing test.

Small sensors placed on the head record electrical activity produced by the hearing pathway. Earphones deliver repeating tones at different pitches and loudness levels, while a computer analyses whether the brain's response follows each sound pattern. The test estimates the quietest levels that produce a detectable response in each ear.

The result can help describe the degree and pattern of hearing loss and guide hearing-aid planning. It is an estimate rather than a diagnosis by itself, so an audiologist interprets it with ear examination, middle-ear checks, other objective tests and behavioural results when available.

The body needs to remain quiet because movement can obscure the small electrical signals. Babies may be tested during natural sleep; some children or adults may need sedation under a service's clinical protocol. Follow the clinic's instructions about sleep, food and medicines exactly, and do not give a sedative unless the clinical team has prescribed and supervised it.

Tell the audiology team about recent ear infections, ear surgery, hearing devices and medicines. Earwax buildup or middle-ear fluid can affect sound reaching the inner ear, so these findings matter when the results are interpreted. The sensors record activity; they do not deliver electricity into the head.

Seek urgent medical assessment for hearing that suddenly drops in one or both ears, especially with new ringing, dizziness or a blocked-ear sensation. Do not wait for a routine ASSR appointment, because sudden hearing loss may need time-sensitive evaluation.

Emergency care is appropriate when hearing change follows a head injury or occurs with facial weakness, severe headache, confusion, difficulty speaking or loss of balance. Ear pain with fever, discharge or swelling around the ear also warrants prompt clinical advice.

Ask what hearing levels were estimated for each ear, whether the pattern suggests conductive, sensorineural or mixed hearing loss, and whether any movement affected test quality. Confirm which follow-up tests are needed and how the findings may influence communication support or hearing-device choices.
