Hip Arthrocentesis: Purpose, Process and Aftercare

Because the joint lies beneath thick muscles and close to important nerves and blood vessels, ultrasound or X-ray guidance helps the clinician plan a safe route and confirm the needle position.

Hip fluid may be assessed for its appearance, cell pattern, crystals and microorganisms. These findings are interpreted alongside symptoms, blood tests, imaging and examination. Aspiration is particularly useful when clinicians need to distinguish joint infection from other causes of a painful, inflamed hip.

Removing fluid may ease pressure, but diagnostic hip aspiration is not a cure for the process producing it. Medicine is not automatically injected afterward. If infection is possible, placing a steroid into the joint may be inappropriate until the diagnosis is clearer.

Provide a current medicine list and mention blood thinners, allergies, fever, skin infection near the groin, recent antibiotics and any hip replacement. Follow the service's instructions about eating, drinking and medicines, as these depend on whether sedation or another procedure is planned. Do not stop prescribed medication without specific advice.

During the procedure, the skin is cleaned and local anaesthetic may numb the route. After the sample is taken, follow dressing and activity guidance and arrange transport if sedation was used. Confirm who will explain the laboratory findings and whether treatment must begin before every result is available.

A rapidly worsening painful hip with fever, chills, inability to bear weight or feeling seriously unwell requires urgent assessment. These symptoms can accompany joint infection, which needs timely diagnosis and treatment.

Following aspiration, obtain prompt medical help if the puncture site develops spreading redness, increasing warmth, persistent drainage or escalating pain. Emergency care is appropriate for breathing difficulty, facial swelling, collapse, or a newly cold, pale or numb leg.

Ask what diagnosis the sample is intended to clarify, which imaging method will guide the needle and whether local anaesthetic or sedation is planned. It is also useful to ask which fluid tests are being requested, when preliminary information may be available and what happens if too little fluid can be obtained.
