Apremilast for lichen planus: what patients should know

It is an oral medicine that alters inflammatory signalling rather than broadly suppressing the immune system. A dermatologist might discuss it when disease is persistent, widespread or insufficiently controlled with local treatment. Its use for lichen planus may be outside the medicine's formally approved indications, depending on local authorization and the precise diagnosis.

Lichen planus can affect skin, mouth, scalp, nails or genital tissue, and treatment choices differ by site and severity. Apremilast tablets act on an enzyme involved in inflammatory pathways. Improvement, when it occurs, is usually assessed through changes in itch, discomfort, new lesions and healing of existing areas rather than an immediate response after a dose.

Stomach upset, diarrhoea, nausea and headache can occur, particularly near the start of treatment. Reduced appetite or unintended weight loss also deserves attention. Mood changes are uncommon but clinically important, so a history of depression or suicidal thoughts should be discussed before starting.

Confirm that the diagnosis and affected body sites have been examined, because oral, genital, scalp and nail disease may need additional care. Give the prescriber a complete medicines list and disclose kidney problems, pregnancy plans, breastfeeding, low body weight and any mental-health history. Some medicines can reduce apremilast exposure and make it less effective.

Take only the prescribed schedule; treatment commonly begins with gradual dose escalation to limit digestive effects. Do not double a missed dose. Track bowel symptoms, appetite, weight, mood and changes in lesions, and agree in advance how the dermatologist will judge whether continuing is worthwhile.

Seek urgent help for swelling of the face or throat, trouble breathing, faintness, or thoughts of self-harm. Contact the prescribing clinic promptly for severe or persistent diarrhoea or vomiting, inability to keep fluids down, marked weight loss, a major mood change, or rapidly worsening painful mouth or genital sores. Do not stop or restart prescribed treatment without discussing the reason, unless emergency clinicians direct you otherwise.
