Diagnosis usually requires imaging and a carefully planned biopsy. Treatment is coordinated by a specialist bone-tumour team and commonly combines systemic chemotherapy with surgery to remove the tumour while preserving useful limb function when feasible.

Osteosarcoma cells produce abnormal bone-like material. Appendicular describes the location in an arm or leg rather than a separate cell type. Pain that persists, worsens or interrupts sleep and swelling around the affected bone are recognised presentations; reduced joint movement or a limp can also occur.

The team assesses the full affected bone and nearby tissues, and checks other parts of the body for spread. The biopsy route matters because it must be positioned so that it can be removed during definitive surgery. For that reason, biopsy planning should involve the centre that would perform tumour surgery.

Expect review by specialists in orthopaedic oncology, medical oncology, radiology, pathology and rehabilitation. They use the tumour's site and extent, biopsy findings, response to treatment and your general health to shape the plan.

Protect the affected limb from heavy impact until the team explains what weight-bearing is safe, because a tumour can weaken bone. Take medicines as prescribed, report treatment side effects early and keep rehabilitation appointments. Physiotherapy and occupational therapy can help prepare for changes in strength, movement and daily activities.

Seek urgent help for sudden severe limb pain, a new deformity, loss of feeling, a cold or pale hand or foot, or inability to use the limb after minor force. During chemotherapy, follow the team's urgent instructions for fever, shaking chills, uncontrolled vomiting, unusual bleeding, chest pain or breathlessness.

Ask who will plan the biopsy, whether the joint, nerves or blood vessels are involved, and what surgical choices are technically possible. Discuss the goals and timing of chemotherapy, expected effects on mobility, fertility-preservation options where relevant, rehabilitation needs and the schedule for monitoring treatment response.
