Can appendicitis cause groin pain?

The location can vary because the appendix does not sit in exactly the same position in everyone. A clinician considers the pain pattern together with abdominal tenderness, nausea, vomiting, appetite loss, fever, and changes in bowel or urinary symptoms.

What the pain pattern may mean

Appendicitis often presents with abdominal pain that becomes more focused on the lower right side. When the appendix lies lower in the pelvis, nearby tissues may be irritated and discomfort may be felt closer to the groin. Movement, coughing, or bumps during travel may aggravate abdominal pain.

Pain centered in the groin can also arise from a muscle or tendon injury, a hip problem, a hernia, the urinary tract, or reproductive organs. The symptom alone cannot identify its cause, and appendicitis should not be inferred from isolated groin discomfort.

What to do now

Arrange prompt medical assessment for new or worsening lower abdominal pain, particularly when it is accompanied by nausea, vomiting, fever, appetite loss, or marked tenderness. Note where the pain began, whether it moved, and which symptoms appeared with it.

Until assessed, avoid strenuous activity and do not press repeatedly on the painful area. Do not use laxatives for unexplained abdominal pain. A medical examination may include abdominal assessment and, when appropriate, urine, blood, or imaging tests.

When to seek urgent care

Go to an emergency department for severe or rapidly worsening abdominal pain, a rigid or very tender abdomen, repeated vomiting, fainting, confusion, difficulty breathing, or feeling seriously unwell. Urgent care is also appropriate when pain briefly eases and then returns more widely across the abdomen.

Sudden severe groin or testicular pain, a painful groin swelling that cannot be pushed back, or pain during pregnancy also needs urgent assessment because causes other than appendicitis may require immediate treatment.
