Anti-tetanus injection: what patients should know

The right treatment depends on the wound and your vaccination history. The term may refer to a tetanus-containing vaccine, while tetanus immunoglobulin is a different medicine sometimes used for immediate short-term protection in higher-risk situations.

A tetanus-containing vaccine trains the immune system to recognise the toxin produced by tetanus bacteria. It is used as part of routine vaccination and may also be offered after an injury when protection is incomplete, uncertain or due for updating.

Tetanus immunoglobulin supplies ready-made antibodies. It is not a substitute for completing the vaccine course, and not every cut or puncture needs it. A clinician considers contamination, damaged tissue, the type of injury and previous doses before recommending either product.

Rinse the wound under clean running water, gently wash around it with soap, remove obvious loose debris and apply a clean covering. Do not pour harsh chemicals into the wound. Arrange an assessment if it is deep, punctured, crushed, contaminated with soil or animal waste, caused by a bite, or contains tissue that looks badly damaged.

Take your vaccine record if available and explain when and how the injury occurred. After vaccination, mild soreness, redness or swelling at the injection site can occur. A cool compress and gentle arm movement may help; a pharmacist or clinician can advise on suitable pain relief for you.

Seek urgent care if bleeding continues despite firm pressure, the wound exposes deeper tissue, sensation or movement is reduced, or a foreign object remains embedded. Increasing redness, warmth, swelling, discharge, fever or worsening pain can signal infection. Jaw or muscle stiffness, painful spasms, difficulty swallowing or breathing, and a severe allergic reaction after an injection require emergency assessment.

Confirm which vaccine you are receiving, whether it includes protection against other infections, and whether further doses are needed. If the injection follows an injury, ask whether tetanus immunoglobulin, antibiotics, stitches or specialist wound care are also appropriate. Mention previous severe vaccine reactions, medicines that affect immunity and any condition that changes your vaccination plan.
