The short answer

Extra body fluid, pressure from the growing uterus on leg veins, heat, and long periods upright can all make fluid collect around the feet and ankles. A familiar pattern that builds through the day and improves with rest is generally less concerning than a rapid change.

Why it happens late in pregnancy

Pregnancy increases the amount of fluid in the body. As the uterus enlarges, it can slow the return of blood from the legs, allowing fluid to move into nearby tissues. The ankles often look puffier after standing, sitting still, or spending time in hot weather.

Swelling alone cannot show whether blood pressure is normal. Routine maternity checks remain important because pre-eclampsia may first be detected through blood-pressure and urine testing, even when a person feels well.

Ways to reduce discomfort

Rest with your feet supported, change position regularly, and take gentle walks if your maternity team has not restricted activity. Flexing and circling the ankles while seated can keep the calf muscles moving. Choose roomy, supportive shoes and socks that do not leave a tight band.

Do not start water tablets or herbal remedies for swelling unless your maternity clinician recommends them. Notice whether the swelling affects both sides, how quickly it appeared, and whether rest changes it; these details help your clinician assess the pattern.

When to contact your maternity team

Contact your maternity unit promptly for a sudden increase in swelling, especially if your face or hands also swell, or if you develop a severe headache, visual changes, pain beneath the ribs, vomiting, or a strong sense that you are unwell. These features require assessment for pre-eclampsia.

Urgent assessment is also appropriate when one leg becomes distinctly more swollen, warm, red, or painful. Call emergency services for chest pain, coughing blood, fainting, or sudden difficulty breathing, because these can accompany a clot affecting the lungs.
