The short answer

Swelling in both ankles can be noticeable after birth as the body clears fluid retained during pregnancy or given during labour. The pattern and accompanying symptoms matter more than ankle size alone.

Why it can happen after birth

Pregnancy changes circulation and encourages fluid to collect in the lower legs. After delivery, that extra fluid does not disappear immediately. Long periods of sitting or standing can make it pool around both feet and ankles, while intravenous fluids received during labour may add to temporary puffiness.

Even swelling on both sides that slowly settles is generally more reassuring than a new difference between the legs. One-sided calf or leg swelling with pain, tenderness, warmth, or skin-colour change can be a presentation of deep vein thrombosis and needs prompt assessment.

What may help

Raise your feet on pillows when resting or feeding, wear roomy footwear, and avoid socks or clothing that leave a tight band. Gentle walking and regular ankle movements can support circulation if your maternity team has said activity is appropriate after your delivery.

Drink according to thirst and follow any fluid advice given for your medical conditions. Do not start water tablets or use another person's medicine. Contact your maternity team or clinician if the swelling keeps worsening, interferes with walking, or does not begin to ease.

When to seek care

Seek emergency help now for chest pain, sudden shortness of breath, coughing blood, fainting, or severe difficulty breathing. These can accompany a clot that has travelled to the lungs and should not be watched at home.

Arrange urgent same-day assessment for swelling, pain, warmth, redness, or tenderness mainly in one leg. Also contact your maternity team urgently for a severe or persistent headache, vision changes, pain high in the abdomen, vomiting, sudden swelling of the face or hands, or feeling markedly unwell; postpartum pre-eclampsia can develop after delivery, including after normal blood pressure during pregnancy.

Tell your clinician when the swelling began, whether it affects one or both sides, and whether the calf is painful, warm, or tender. Ask whether your delivery, medicines, blood-pressure history, or reduced mobility changes the need for an examination, blood-pressure check, urine test, or leg scan.
