Ankle swelling after an insect bite

A local reaction often centres on a visible bite or sting and may itch, feel warm and enlarge before settling. Infection and a serious allergic reaction have different warning signs, so check the skin as well as how you feel overall.

Inflammatory chemicals released after a bite can make nearby tissue leaky, producing a raised, itchy area and swelling around the ankle. Scratching can break the skin and allow bacteria to enter. Infection may present with progressively expanding redness, greater pain, heat, pus, red streaking or swollen glands.

A severe allergic reaction affects more than the bite site. Sudden swelling of the lips, tongue or throat, trouble breathing or swallowing, widespread raised rash, dizziness, confusion or collapse signals an emergency, even if ankle swelling seems minor.

Move away from the insect, then wash the skin gently with soap and water. If a visible sting remains, scrape it sideways rather than squeezing it. Apply a wrapped cold pack for short periods, elevate the ankle and avoid scratching. Keep nails short and cover broken skin with a clean dressing.

A pharmacist can advise on an antihistamine, itch treatment or pain relief after checking your medicines and health conditions. Do not apply strong chemicals, cut the skin or try to suck out venom. Marking the edge of redness can help you notice genuine spread.

Call emergency services for breathing difficulty, throat tightness, tongue or lip swelling, fainting, severe dizziness, confusion or blue, grey or unusually pale skin. Use a prescribed adrenaline auto-injector promptly if your allergy plan instructs you to, then still obtain emergency care.

Seek same-day medical advice if redness or swelling is rapidly expanding, the skin becomes increasingly hot or painful, pus appears, a red line travels up the leg, or you develop fever or swollen glands. Bites near the eye or inside the mouth also need prompt advice.

Ask whether the pattern is a local allergic response or infection, which treatments are suitable, and how long to monitor the swelling. After a severe reaction, discuss allergy referral, an emergency action plan and whether you should carry adrenaline.
