How to interpret post-flight swelling

Sitting still slows the return of blood and tissue fluid from the lower legs. Gravity then allows fluid to gather around the ankles. Symmetrical puffiness that improves with walking and elevation is less concerning than a new, painful difference between the legs.

Fluid pooling versus a blood clot

Temporary fluid pooling often affects both ankles and may leave a shallow indentation after pressure. A deep vein thrombosis is a clot in a deep leg vein and can present with swelling in one leg, calf or thigh pain, warmth, tenderness or skin colour change. It may also occur without obvious symptoms.

Recent surgery, previous clots, active cancer, pregnancy, reduced mobility and some medicines can alter clot risk. These factors do not diagnose a clot, but they lower the threshold for medical assessment when new one-sided symptoms appear.

Steps for mild, even swelling

Walk gently when safe, move the ankles through a comfortable range, and elevate the legs while resting. Drink normally according to your usual medical advice. Avoid tight bands around the calves or ankles and compare both legs for differences in colour, warmth, pain and size.

Do not massage a leg that is newly swollen on one side or painful, because a clot must first be excluded. Compression stockings require the correct size and are not suitable for everyone; seek advice if you have circulation problems, skin damage or unexplained swelling.

Urgent warning signs

Get urgent medical help for new swelling mainly in one leg, especially with calf pain, warmth, tenderness or skin discolouration. A clinician may need to assess the veins rather than relying on appearance alone.

Call emergency services immediately for sudden shortness of breath, chest pain, coughing blood, fainting, a very fast heartbeat or severe weakness. These can accompany a clot that has travelled to the lungs. Persistent swelling in both legs also deserves review if it is accompanied by breathlessness or reduced exercise tolerance.

Discuss whether your history changes your clot risk, whether travel compression is appropriate, and how often you should move during a future flight. If swelling recurs, ask whether heart, kidney, vein, lymphatic or medicine-related causes need evaluation.
