What should you know about ankle osteomyelitis?

Short answer
Ankle osteomyelitis is an infection in a bone around the ankle that requires timely medical assessment and targeted treatment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance

- Infection within bone near the ankle

- Examination, blood tests, imaging and sometimes a tissue sample
- Treatment focus
- Control infection and remove unhealthy tissue when necessary
The short answer
Microbes may reach bone through the bloodstream, an open wound, nearby infected tissue, or after an injury or operation. Treatment commonly uses antibiotics selected for the organism; some cases also need surgery to drain infection or remove unhealthy tissue.
How a bone infection can appear
Osteomyelitis can cause focused bone pain, warmth, swelling and redness around the ankle. Fever, chills, fatigue, drainage from a wound or a wound that does not heal may accompany it, but systemic symptoms are not always present. A deeper or slow-moving infection may produce persistent pain without dramatic skin changes.
Clinicians consider recent wounds, ulcers, surgery, fractures, implanted hardware and infections elsewhere in the body. Blood tests can look for inflammation and infection, while imaging evaluates bone and nearby soft tissues. Blood, wound or bone samples may be needed to identify the organism and select an effective medicine.
What diagnosis and treatment involve
Do not squeeze or probe a draining wound. Cover it with a clean dressing, reduce pressure on the area and obtain medical care. Tell the clinician about diabetes, circulation problems, immune-suppressing medicines, recent antibiotics, operations and any metalwork in the ankle, because these details affect investigation and treatment planning.
Complete antibiotics exactly as prescribed and attend planned blood tests, wound checks and imaging. Surgery may be required when there is an abscess, dead bone, infected hardware or tissue that prevents antibiotics from controlling the infection. When removal leaves a bone defect, reconstruction may be discussed after infected tissue has been addressed.
Warning signs that need urgent care
Seek urgent assessment for a hot, swollen and increasingly painful ankle with fever or chills; pus or a bad-smelling discharge; spreading redness; confusion; marked weakness; or rapid deterioration. A wound that exposes bone or follows a puncture near the ankle also needs prompt medical attention.
People with diabetes, reduced sensation, poor circulation or a weakened immune system should arrange early review for any new ankle wound, colour change or unexplained swelling. Lack of pain does not make a wound harmless when sensation is impaired.
Points to clarify with your care team
Ask which bone and surrounding tissues are involved, whether a sample has identified the organism, and how treatment response will be monitored. Confirm how to care for the wound, whether weight bearing is permitted, what medicine effects need attention and whether surgical cleaning or removal of hardware is being considered.

Questions people ask
Can ankle osteomyelitis occur without a fever?
Yes. A bone infection may present with persistent local pain, swelling, warmth or a non-healing wound without fever, particularly when it develops gradually or immune responses are reduced.
Why might a bone sample be needed?
A bone or deep-tissue sample can identify the organism more reliably than a surface swab in some cases. This helps the clinical team choose targeted antimicrobial treatment.
Can antibiotics alone treat the infection?
Antibiotics may be sufficient in selected cases, but an abscess, dead bone, unstable tissue or infected hardware may require surgery as well. The decision follows examination, imaging and sample results.
