The short answer
It can follow a sudden push-off, awkward step, rapid change of direction or repeated loading. Because ankle pain can also come from a ligament sprain, tendon injury or fracture, the exact injured structure is not always clear from symptoms alone.
What the injury means
Muscles that cross or support the ankle help control foot movement and balance. A strain occurs when muscle fibres are loaded beyond what they can tolerate. Pain often increases when the affected muscle contracts or is gently stretched, and bruising or stiffness may develop.
A strain differs from an ankle sprain, which affects ligaments connecting bones. Pain location, the movement that triggered it and the ability to bear weight help a clinician decide whether examination or imaging is appropriate.
What to do after the injury
Stop the activity that caused sharp pain and protect the ankle from movements that worsen symptoms. Relative rest is usually more useful than complete immobility: keep comfortable movement in the toes and ankle, but do not force a painful range.
A wrapped cold pack may ease pain for short periods, while gentle compression and elevation can help with swelling if they feel comfortable. Compression should not cause numbness, tingling, colour change or increasing pain. As walking becomes easier, gradually restore motion and strength; a physiotherapist can guide loading, balance work and return to sport.
When to seek medical care
Seek urgent assessment after a major injury if the ankle looks deformed, you cannot take steps, pain is severe, or you heard a snap and then lost strength. Prompt care is also needed for a foot that becomes cold, pale or blue, or for new numbness.
Arrange an assessment if swelling or bruising is substantial, pain is focused directly over a bone, symptoms keep worsening, or function is not steadily returning. Increasing redness, warmth, fever, or calf swelling and tenderness also warrant medical review.
