How alexithymia and ADHD can overlap

ADHD involves persistent patterns of inattention and/or hyperactivity-impulsivity that interfere with daily functioning. Alexithymia is a trait involving emotional awareness and communication rather than a standalone ADHD symptom. Either may be present without the other.

Possible day-to-day signs include giving broad answers such as “fine,” recognizing an emotion only after it becomes intense, confusing anxiety with physical illness, or finding emotionally focused conversations exhausting. Similar experiences can also accompany anxiety, depression, autism, trauma, sleep problems, or medication effects, so self-observation alone cannot settle the cause.

Practical ways to build emotional awareness

Try brief check-ins that start with physical clues: breathing, jaw tension, temperature, energy, and the urge to move away or act. Then choose from a short list of emotion words and note what happened beforehand. A phone reminder or simple diary can make this easier when attention is inconsistent.

Use direct communication with trusted people, such as asking for time to process before responding. A psychologist or other qualified clinician can assess ADHD symptoms, emotional awareness, mood, anxiety, development, and functioning. Support may include ADHD treatment, skills-based therapy, emotion vocabulary work, or adjustments to routines, depending on the assessment.

When to seek professional support

Book an assessment if difficulties recognizing emotions repeatedly disrupt relationships, work, education, treatment decisions, or self-care. Prompt review is also sensible when irritability, anxiety, low mood, sleep change, or impulsive behaviour is worsening.

Seek urgent help if you may harm yourself or someone else, feel unable to stay safe, or experience a severe mental health crisis. Contact local emergency services or go to the nearest emergency department, and involve a trusted person if possible.
