Age spot on the forehead: next steps

Many benign pigment changes occur on the forehead, and location does not establish what a mark is. Confirmation is particularly useful before applying strong lightening agents to visible facial skin.

The forehead receives regular ultraviolet exposure, including along the hairline where sunscreen may be missed. Solar lentigines tend to be flat, tan or brown, and more clearly bordered than surrounding skin.

Broader patches can be related to a different pigment pattern, while a mark left by a blemish may follow inflammation. Hair products, hats and active skincare can also irritate the area. A clinician considers distribution, texture, colour pattern and history rather than relying on shade alone.

Extend broad-spectrum sunscreen to the hairline and temples, and add a brimmed hat and shade during outdoor exposure. Wash away sweat gently instead of rubbing the spot, and keep irritating hair or facial products off inflamed skin.

If a dermatologist confirms an age spot, treatment may involve a prescribed topical agent, a peel, freezing, intense pulsed light or a selected laser. The clinician should account for your complexion, recent tanning, tendency toward post-inflammatory pigmentation and whether the mark sits near the eyebrows or hairline.

Arrange an examination for a mark that is new and evolving, asymmetrical, irregularly bordered, unevenly coloured, thickening, scaly, persistently itchy, tender, bleeding or failing to heal. Rapid visible change is a reason to seek care rather than experiment with cosmetic removal.

After treatment, contact the treating clinic for worsening pain, extensive blistering, spreading redness, pus, fever, eye swelling or an unexpected wound. Mild temporary redness may occur with some procedures, but escalating symptoms need individual advice.

Ask whether the mark fits a solar lentigo or another diagnosis, whether any product should be stopped beforehand, and how the eyebrows and hairline will be protected. Discuss the likely skin response, aftercare, possible pigment changes and how recurrence or new spots will be managed.
