What a self-assessment can show
A questionnaire asks about patterns such as losing track of tasks, forgetfulness, restlessness or acting before considering consequences. Its score may indicate that a fuller conversation is worthwhile. It cannot establish when the pattern began, confirm its presence across settings or determine how much it impairs daily function without additional context.
Attention difficulties can also occur with poor sleep, anxiety, low mood, substance use, learning differences, physical illness or medicine effects. ADHD may coexist with some of these concerns. For that reason, a clinician interprets symptoms alongside health history, development and day-to-day impact rather than relying on a checklist result alone.
How to use self-assessment information
Answer according to your usual experience rather than an unusually good or difficult day. Record concrete examples, including the setting, what you intended to do, what occurred and the consequence. If possible, note whether similar patterns were present earlier in life and whether someone who knows you well has observed them.
Take the completed form and examples to a healthcare appointment instead of changing treatment yourself. Bring a medicines list and mention sleep, mood, alcohol or other substance use, injuries and relevant medical conditions. The professional may ask for further information before discussing whether the findings fit ADHD or another explanation.
When to arrange care
Consider an evaluation when attention, organisation, restlessness or impulsive behaviour repeatedly disrupts work, study, relationships, finances, driving or self-care. A sudden new problem with concentration needs medical review because ADHD begins as a developmental pattern rather than appearing abruptly. Seek urgent support for thoughts of self-harm, hallucinations, severe confusion or behaviour that creates immediate danger.

You may also ask who can complete a formal evaluation, how your current medicines affect attention and what practical support is reasonable while assessment is underway.
