The short answer
It asks about patterns such as losing track of tasks, forgetfulness, restlessness, impulsive actions, and difficulty organising activities. Depending on its purpose, it may be completed by the person, a parent, a teacher, or another observer. Results guide a conversation about whether fuller clinical assessment is appropriate.
What the answers can and cannot show
A questionnaire captures reported experiences during a defined period or setting. Similar answers can occur with anxiety, depression, poor sleep, trauma, learning difficulties, autism, substance use, medicine effects, or physical illness. ADHD assessment therefore considers when traits began, whether they occur across settings, how they impair daily life, and what other explanations may apply.
A low score does not settle the question when language, masking, misunderstanding, or an unsuitable form affects responses. A high score indicates features needing interpretation rather than proving ADHD.
How to answer usefully
Choose responses that reflect your usual functioning rather than an unusually easy or difficult day. Think of concrete examples from home, education, work, relationships, and routine responsibilities. If wording is unclear, note that instead of guessing what the assessor wants.
Take the completed form to a qualified clinician with a medicine list, relevant health details, and any records requested. Explain strategies you use to compensate, because calendars, reminders, or support from others may reduce visible problems while requiring substantial effort.
When to seek professional help
Arrange an assessment when attention, organisation, restlessness, or impulsivity persistently interferes with learning, work, relationships, finances, driving, or self-care. Do not wait for questionnaire results when there is immediate danger. Suicidal thoughts, self-harm, severe confusion, extreme agitation, or inability to stay safe require urgent mental health or emergency support.
