Night-time wetting is involuntary. Attention, routines, sleep patterns and difficulty noticing body signals may complicate toileting for some people with ADHD, but ADHD is not the only possible explanation. Constipation, urinary problems, diabetes, sleep disorders, stress and developmental factors may need consideration.

The clinician will want to know whether the person has always wetted at night or started again after a dry period. Daytime urgency, pain, unusual thirst, bowel habits, snoring, sleep quality, medicines and stressful changes provide useful clues. A urine test or other checks may be recommended according to the history and examination.

Punishment and shame do not address the cause and can increase distress. Bed wetting is not evidence of laziness or deliberate defiance. ADHD-related forgetfulness may affect evening routines, so simple, consistent cues are often more workable than repeated verbal reminders.

Supportive steps at home

Keep a calm diary of wet and dry nights, daytime urination, bowel movements, drinks, medicines and relevant sleep symptoms. Encourage regular toilet visits and using the toilet before sleep. Maintain normal daytime hydration; do not severely restrict fluids unless a clinician specifically advises it.

Use washable bedding protection and discreet access to clean clothes. Make the bedtime sequence visible with a short checklist or phone reminder. Discuss alarms or medicines with a clinician because suitability depends on age, motivation, sleep, health history and the pattern of wetting.

When medical review is needed

Arrange prompt care for pain or burning with urination, fever, blood in urine, new daytime accidents, marked thirst, weight loss, leg weakness, altered sensation, or wetting that begins suddenly after a dry period. Severe illness, inability to pass urine, unusual drowsiness or vomiting with dehydration warrants urgent local care.
