The short answer

The pain can begin abruptly, become intense even at rest and make lifting the arm difficult. Calcium on an image does not always explain a person's symptoms, so clinicians match the scan finding with the pain location, movement pattern and examination.

Why the shoulder can flare

Calcific tendinitis involves calcium material within a rotator cuff tendon, commonly near where the tendon attaches to the upper arm bone. A painful inflammatory reaction can occur around the deposit, sometimes while the body is breaking it down. This may irritate the nearby bursa and sharply restrict movement.

The condition often presents with shoulder pain and pain when lifting the arm, but those symptoms also occur with other shoulder problems. Pain may spread toward the upper arm and disturb sleep. Examination helps separate tendon-related pain from stiffness, injury, neck-related nerve symptoms or infection.

Managing pain while protecting movement

During a severe flare, reduce overhead work and heavy lifting, support the arm in a comfortable position and use a wrapped cold pack if soothing. Keep the elbow, wrist and hand moving, and gently move the shoulder within tolerance rather than forcing through sharp pain. Prolonged immobilisation can contribute to stiffness.

A clinician can advise whether pain-relieving or anti-inflammatory medicine is suitable. Physiotherapy may help restore comfortable movement and later rebuild shoulder capacity. If symptoms remain substantial, options can include a cortisone injection for nearby inflammation or shockwave therapy; the choice depends on the clinical findings and the deposit. Shoulder arthroscopy is generally considered only when persistent symptoms and function justify a surgical discussion.

When to get medical help

Obtain urgent assessment if shoulder pain is accompanied by fever, a hot or red joint, rapidly increasing swelling or feeling very unwell. Seek immediate care after a major injury when the shoulder appears deformed, the arm is suddenly weak or numb, or the hand becomes cold or unusually pale.

Arrange a prompt appointment when severe pain prevents sleep or basic self-care, movement is becoming more restricted, or symptoms are not easing. Chest pressure, breathlessness, sweating or pain spreading from the chest to the shoulder or arm should be treated as an emergency rather than assumed to be a tendon problem.
