Abdominal rectus muscle separation explained

Short answer
Abdominal rectus muscle separation, or diastasis recti, is widening and thinning of the connective tissue between the paired front abdominal muscles.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Also called
- Diastasis recti or abdominal separation.
- Assessment focus
- Function and tissue tension matter alongside the visible gap.
- Exercise cue
- Exhale with effort and reduce movements that cause marked doming.
What diastasis recti describes
It can follow pregnancy or other prolonged pressure on the abdominal wall. The muscles have not torn apart, and appearance alone does not show how well the trunk functions.
What changes in the abdominal wall
The linea alba, a band of connective tissue running down the midline, becomes stretched and transfers load less effectively. You may notice a ridge or doming when sitting up, a soft gap around the navel, reduced trunk control, or difficulty coordinating the abdomen during lifting.
A gap can exist without pain or disability. Conversely, a person with a modest visible gap may still have back discomfort, pelvic-floor symptoms, or trouble managing pressure. Assessment therefore considers tension, movement, breathing, strength, and daily function rather than width alone.
Building function safely
Begin with comfortable breathing and controlled abdominal engagement during everyday movements. Exhale during effort, roll onto your side to rise from bed, and reduce an exercise if it causes marked doming, pain, breath-holding, or pelvic heaviness. Gradual strengthening can include the trunk, hips, and pelvic floor.
Postnatal physiotherapy can check movement technique and tailor progression to your symptoms and goals. There is no single exercise that suits everyone, and avoiding all abdominal exercise can delay useful strength recovery.
When an examination is needed
Arrange an assessment if the separation is painful, daily tasks feel unstable, exercise repeatedly causes pronounced bulging, or you also have urine leakage, pelvic pressure, or persistent back pain. A clinician or physiotherapist can also distinguish diastasis from a hernia.
Seek urgent care for a new abdominal lump that becomes severely painful, firm, discoloured, or impossible to push back, particularly with vomiting or abdominal swelling. Those features may indicate a trapped hernia rather than uncomplicated muscle separation.
Questions for a tailored recovery plan
Ask how well the midline creates tension, whether a hernia is present, which movements need temporary modification, and how to progress lifting or exercise without worsening your symptoms.

Questions people ask
Is diastasis recti a muscle tear?
No. The midline connective tissue becomes widened and thinned while the paired rectus muscles sit farther apart. A separate hernia can coexist and requires its own assessment.
Should I avoid abdominal exercises?
Usually, complete avoidance is unnecessary. Choose exercises you can perform with steady breathing, controlled pressure, and no troublesome pain or bulging, then progress according to function.
Can physiotherapy close the gap?
Physiotherapy mainly aims to improve strength, pressure control, movement, and symptoms. The visible distance may change, but function is a more useful recovery target than a particular gap size.
