Ab muscle separation: understanding the midline gap

Short answer
Ab muscle separation is a common name for diastasis recti, where the tissue joining the abdominal muscles widens and may bulge under pressure.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Clinical name
- Diastasis recti
- More than width
- Tissue tension, pressure control, symptoms, and function also matter
- Movement cue
- Breathe out during effort and scale loads to maintain control
The short answer
It is frequently noticed during or after pregnancy, especially as a ridge along the centre of the abdomen when getting out of bed or lifting. The muscles have not torn apart; the central tissue has stretched.
What to notice besides the gap
The distance between the muscles is only one part of the picture. A useful assessment also considers how firm the midline feels when you engage, whether the abdomen can contain pressure during movement, and whether pain, back fatigue, pelvic-floor symptoms, or reduced confidence are present.
The shape can change with posture, breathing, effort, and the stage of recovery after birth. Repeatedly checking or pressing the gap does not show how your whole trunk performs. A focused movement assessment gives more practical information.
Making daily movement more manageable
Roll onto your side before rising from bed if sitting straight up causes pronounced ridging. For lifting, bring the load close, breathe out during the effort, and choose a weight you can manage without holding your breath or straining. These adjustments are tools for comfort, not lifelong restrictions.
Practise gentle trunk engagement while continuing to breathe, then add resistance, reach, rotation, and impact gradually. If you cannot find a comfortable starting point, postnatal physiotherapy can tailor exercises to childcare tasks, work, sport, scar recovery, and pelvic-floor function.
When professional input helps
Arrange an assessment when the bulging is painful, daily tasks feel unstable, exercise consistently worsens symptoms, or you also experience bladder leakage, pelvic pressure, painful sex, or persistent back discomfort. A clinician can look for other causes and plan coordinated rehabilitation.
A sharply painful local lump, skin colour change over a bulge, vomiting, or worsening abdominal swelling needs urgent medical attention. These are not expected features of simple muscle separation and may indicate a hernia complication or another abdominal problem.
What to discuss with a clinician
You might ask how breathing, rib position, pelvic-floor coordination, and lifting habits affect your symptoms; whether a hernia is present; which activities can continue; and what signs will show that training is ready to progress.

Questions people ask
Can ab muscle separation improve after pregnancy?
The abdominal wall can regain tension, strength, and better movement control over time. Progress varies, so function and symptoms are often more useful guides than gap width alone.
Are sit-ups forbidden with abdominal separation?
There is no universal forbidden list. The right version and timing depend on pressure control, comfort, healing, and training goals; exercises can be modified and advanced as capacity improves.
