Lump on the Back of the Neck Near the Spine

The bony bumps of the neck are covered by skin, fat, fascia and muscles. A visible or palpable lump may be a skin cyst, an inflamed hair follicle, a fatty mass, muscle thickening or another soft-tissue change. It may feel as though it sits on a vertebra even when it is above the bone. A neck lump can be a presentation of head and neck cancer, lymphoma, thyroid cancer, or cancer of unknown primary; touch alone cannot distinguish these possibilities from benign causes.

Describe whether the swelling is in the centre or to one side, whether the skin is red, and whether neck movement changes its size or pain. These details help separate a superficial problem from a deeper one.

Make an appointment when the lump lasts, enlarges, becomes hard, or cannot be linked to a recent injury or skin irritation. Do not try to drain it or manipulate the neck. Photographing the area and noting changes in tenderness or mobility can help at the visit.

The clinician may inspect the skin and feel the spine, muscles and nearby lymph nodes. Ultrasound or other imaging may be chosen if the depth is unclear; a needle sample can provide cells when needed. A thyroid finding may lead to discussion of Thyroid Removal Surgery only after appropriate assessment.

Seek urgent care after significant neck injury, or for new weakness, numbness, difficulty walking, loss of bladder or bowel control, breathing trouble or inability to swallow. Arrange prompt medical review for a growing or fixed lump, persistent night sweats, unexplained weight loss, fever, draining skin, or pain that is worsening. These warning signs need a clinical assessment; they do not establish a diagnosis by themselves.

Ask whether the lump is in the skin, muscle or deeper tissue and whether the vertebra itself appears involved. Ask if imaging is needed, whether an infection or cyst is plausible, and what symptoms should prompt urgent review. Ask when the result will be available and what the next step would be for each possible finding.
