Lump on the Back of the Neck Hairline

The hairline contains skin, hair follicles, and lymph nodes. A blocked follicle, inflamed cyst, insect bite, or skin infection may cause a surface bump. Lymph nodes in this area can swell when the scalp or nearby ear is inflamed. A persistent mass may sometimes be associated with lymphoma or a head and neck cancer. Location does not identify the diagnosis, and a hairline lump is not, by itself, evidence of cancer.

Look for a central pore, redness, warmth, drainage, or tenderness without picking at the area. Also note whether the lump is beneath the skin, mobile, firm, or attached to deeper tissue.

Arrange a clinical review when the bump does not improve, keeps enlarging, returns repeatedly, or has no clear skin-related explanation. The clinician may inspect the scalp and hairline, feel the neck, and check the ears, mouth, and throat. Testing may include an ultrasound, blood test, or needle sample. Keep the area clean, avoid squeezing it, and bring details of hair products, skin irritation, or recent infection.

Seek urgent help for difficulty breathing or swallowing, sudden voice change, rapidly spreading redness, or severe pain with fever. Arrange prompt review for steady growth, a hard or fixed lump, persistent fever, night sweats, or unexplained weight loss. These findings can have non-cancerous causes, but they merit timely assessment.

Ask whether the lump appears to be in the skin or a lymph node, what signs suggest infection, and whether imaging is needed. Ask how to care for the area while waiting and when to return if it changes. If a deeper neck or thyroid cause is suspected, ask whether a specialist review or thyroid removal surgery could be relevant.
