What should you know about refractory asthma?

People may continue to have wheezing, cough, chest tightness, night waking, or activity limits despite using prescribed preventer treatment. Before asthma is labelled refractory, clinicians usually check the diagnosis, inhaler technique, adherence, triggers, other conditions, and ongoing exposure to irritants. Allergic rhinitis, house dust mite allergy, reflux, and chronic cough can complicate the picture.

The label signals a need for a detailed assessment, not a reason to accept daily symptoms as normal. Asthma severity can change, and treatment should be reviewed when control changes.

Use inhalers according to your written plan and ask a clinician to watch your technique. Track symptoms, reliever use, night waking, triggers, and peak-flow readings if advised. Reduce smoke, strong scents, dust, and other personal triggers where practical. Keep regular reviews so medicines, allergies, and related conditions can be reassessed.

A specialist may consider additional tests or treatments when symptoms persist. Nebuliser treatment is used in selected situations; it is not a substitute for an agreed asthma action plan.

Follow your asthma action plan and seek emergency help if breathing becomes severely difficult, speaking is hard, lips look blue, the reliever is not helping as directed, or symptoms worsen quickly. Sit upright and use prescribed rescue treatment while arranging help. Do not delay because symptoms have happened before.
