PCOS and bleeding after intercourse: what to know

PCOS commonly disrupts ovulation, which can lead to irregular periods, missed periods, or bleeding between periods. That bleeding may happen around the same time as intercourse without intercourse being the cause. Anovulation can also leave the uterine lining exposed to uneven hormone signals.

Bleeding that starts during or soon after sex can instead come from friction or vaginal dryness, irritation of the cervix, a cervical polyp, or an infection such as a sexually transmitted infection. Pregnancy-related bleeding is another possibility. A clinician may need to examine the vagina and cervix and may offer pregnancy or infection testing; PCOS alone does not explain every episode.

Make a note of when the bleeding occurred, how long it lasted, whether it followed deep penetration, and whether you had pain, discharge, itching, or a missed period. Arrange a medical appointment if it happens again or if you have not had a recent cervical check. Until you are assessed, use gentle lubrication and stop if sex causes pain. Do not self-treat presumed infection with leftover medicine.

Tell the clinician about PCOS, cycle changes, contraception, pregnancy possibility, and any new sexual partner. They may review your cycle pattern and discuss options for protecting the uterine lining, such as a Combined Pill when suitable.

Seek urgent medical help for bleeding that is heavy or worsening, severe or one-sided pelvic pain, fainting, marked dizziness, fever, or bleeding when you may be pregnant. These symptoms can need prompt assessment, including for pregnancy complications or infection. Contact a clinician soon for repeated post-coital bleeding, bleeding after menopause, or bleeding with unusual discharge or pelvic pain.
