Coughing Up a Chunk of Blood: What Should You Know?

Blood may come from irritated airways after forceful coughing, an infection such as pneumonia, or a condition that causes widened, mucus-filled airways such as bronchiectasis. Less commonly, a clot can be linked with a serious lung problem, including pulmonary embolism or lung cancer. A blood-stained piece of mucus does not by itself identify the cause.

The appearance matters, but it is not enough to judge severity at home. Fresh bright blood, dark material or a jelly-like piece should all be reported. Blood from the nose, mouth or stomach can sometimes be mistaken for blood from the lungs.

Stop strenuous activity, sit upright and arrange same-day medical advice. Note when it happened, how much material there was, its colour, and whether coughing, fever, chest pain or breathlessness accompanied it. If possible, keep a clear description or photograph for the clinician. Do not take a new medicine to stop bleeding unless a clinician advises it, and tell them about blood thinners.

Call emergency services for more than a small amount of blood, repeated bleeding, severe breathlessness, sharp chest pain, a fast heartbeat, fainting, confusion or blue lips. These symptoms can signal dangerous bleeding or a blood clot in the lungs. Do not drive yourself if you feel faint or short of breath.

Ask where the blood is most likely coming from, whether you need chest imaging or blood tests, and whether infection, airway disease or a clot needs assessment. Share your smoking history, recent travel or immobility, fever, weight change and all medicines.
