What expiratory wheezing can mean
A high-pitched whistling sound during breathing out is produced as air passes through tighter air passages. Asthma may cause episodes after exercise, at night, or around triggers such as dust, smoke, or pollen. A chest infection such as acute bronchitis can temporarily irritate the airways. Allergic rhinitis may accompany breathing symptoms when nasal allergy and airway sensitivity occur together. COPD can cause persistent airflow limitation, especially in people with a history of smoking or other long-term irritant exposure. In babies, bronchiolitis is another possible cause.

Move away from smoke, strong scents, dust, or a known allergy trigger, and rest while you assess your breathing. Use a prescribed reliever inhaler exactly as directed; do not borrow someone else’s medicine. Arrange a clinical assessment if the wheeze is new, keeps returning, lasts beyond a respiratory illness, or affects sleep or activity. A clinician may listen to your chest and decide whether breathing tests, allergy assessment, or treatment such as a nebuliser is appropriate.
When to seek care urgently
Seek emergency help for severe or rapidly worsening breathlessness, blue or grey lips, confusion, fainting, inability to speak in full sentences, or swelling of the lips, tongue, or throat. Sudden wheezing after a food, medicine, or insect sting may signal anaphylaxis and needs immediate treatment. Prompt assessment is also sensible when wheezing comes with chest pain, high fever, or marked weakness.
