Blood-Tinged Sputum in the Morning: What It Can Mean

Bright red streaks may appear after forceful coughing because delicate airway lining has been irritated. Dry air, smoking, a recent infection, or mucus draining overnight from inflamed sinuses can contribute. Acute bronchitis and pneumonia can cause phlegm with blood, while bronchiectasis and COPD may cause recurrent or larger amounts. Chronic sinusitis can leave blood in mucus that is cleared after waking. The colour, amount, frequency, fever, breathlessness and chest discomfort all help a clinician interpret it.

Record when it happens, whether the blood is streaked through mucus or separate, and how much you see. Avoid smoking and vaping, drink fluids if you can, and do not start leftover antibiotics. Arrange a medical review if it returns, lasts beyond a brief episode, or occurs with a persistent cough, fever or wheeze. A clinician may examine your nose, throat and chest and decide whether tests are needed.

Seek emergency help for more than a small trace, bleeding that does not stop, difficulty breathing, chest pain, faintness, confusion, blue lips, or coughing up clots. Prompt assessment is also needed if you feel severely unwell or have a high fever with worsening breathing. Do not drive yourself if you are faint or struggling to breathe.
