Blood cough in the morning: what to know

Mucus and small amounts of blood can collect overnight when a dry nose, allergies or a blocked sinus drains into the throat. Mouth breathing and snoring may leave the throat dry, so the first cough can be forceful. Reflux during sleep can also irritate the throat and contribute to morning coughing. A blood-streaked phlegm pattern may instead arise from bronchial inflammation or another lung problem. Acute Bronchitis, Chronic Cough and Bronchiectasis are examples of airway conditions in which mucus and coughing can occur; the timing alone cannot identify the cause.

Take a photograph or note the colour and amount before cleaning your mouth. Check for a nosebleed, gum bleeding, mouth sore or blood only after brushing. Drink fluids if you can, avoid smoke and do not use someone else’s antibiotics. Arrange a medical review if the finding returns, continues beyond a brief illness, or is accompanied by wheeze, fever or increasing phlegm. Bring a list of medicines, including products that affect clotting.

Call emergency services for significant bleeding, shortness of breath, chest pain, faintness, confusion or blue lips. Seek same-day advice if blood appears on several mornings, follows an injury, or occurs with fever, night sweats, unexplained weight loss or worsening breathing.
