Blood in cough and nose mucus: what to know

A dry nose, forceful nose blowing, postnasal drainage, or frequent coughing can break small surface vessels. A cold and Allergic Rhinitis may inflame the upper airway; Acute Bronchitis can make the lower airways tender and produce streaks in phlegm. Acid Reflux may irritate the throat and trigger coughing. The colour and source matter: bright red flecks on mucus can differ from blood that is mixed deeply into sputum, while blood from the nose may trickle backward and be coughed out later.

Asthma can cause cough and airway irritation, but blood is not something to dismiss as an asthma symptom. Bronchiectasis and other airway problems can also produce blood-stained sputum. The symptom alone cannot identify its cause.

Sit upright, spit out the material so you can observe it, and avoid forceful throat clearing. If your nose is dry or bleeding, gentle pressure and humidified air may help; do not insert objects into the nose. Note when it began, whether it followed a nosebleed, how often it appears, and whether the material is streaked or mostly blood. Arrange a clinical review if it returns, lasts beyond a short illness, or comes with fever, worsening cough, wheeze, or chest discomfort.

Seek emergency help for more than small streaks, ongoing bleeding, breathlessness, chest pain, faintness, blue lips, confusion, or coughing that prevents you from breathing comfortably. Prompt assessment is also sensible if you take a blood-thinning medicine, have known lung disease, or cannot tell whether the blood is from your lungs, nose, or throat.

Ask whether the blood appears to come from the upper airway or lungs, whether your medicines could increase bleeding, and which examination or tests fit your symptoms. Mention smoking or vaping, recent infection, reflux symptoms, allergies, and any established diagnosis such as Chronic Cough or Asthma.
