Blood in cough from the throat: what it can mean

A dry throat, forceful coughing, a nosebleed draining backwards, or inflamed gums can leave small streaks on mucus. Blood from the airways is often mixed with phlegm and may appear after repeated coughing. A cold, acute bronchitis, asthma flare, or chronic cough can irritate these passages. Acid reflux may also irritate the throat and trigger cough. Bronchiectasis can produce recurring mucus and blood. The appearance alone cannot identify the source.

Notice whether the blood is bright red or dark, mixed through mucus or sitting on its surface, and whether it follows nose or mouth bleeding. Also record cough duration, fever, chest discomfort, breathlessness, wheezing, and medicines that affect clotting.

Stop smoking or vaping, sip water, and avoid strenuous coughing or throat clearing. Do not take a cough suppressant or blood-thinning medicine specifically for this symptom without medical advice. Arrange a clinical assessment, particularly if the blood returns, the cough persists, or you have lung disease. A clinician may examine your nose, mouth and chest and decide whether further tests are needed.

Seek emergency help for more than a trace of blood, repeated bleeding, trouble breathing, chest pain, faintness, blue lips, confusion, or a fast worsening condition. Prompt medical review is also needed for fever with deterioration, unexplained weight loss, night sweats, or blood that keeps appearing. Do not wait for a routine appointment if you take anticoagulants or have a known bleeding disorder.

Ask: Could the blood be from my nose or throat? What features suggest an airway source? Which medicines should I continue? Do my cough duration and other symptoms require imaging or a respiratory review?
