A Big Milia-Like Bump

Milia are small, firm, pale bumps formed when keratin becomes trapped beneath the skin surface. A larger lesion may still contain keratin, but a cyst, closed comedone, inflamed spot, or another growth can look similar.

Typical milia feel firm and sit just under an intact surface. They usually lack the central dark opening seen in some comedones and are not surrounded by marked redness. Whiteheads arise from blocked pores and may occur with other acne changes. A deeper cyst is often broader beneath the skin and can become tender or inflamed.

Size alone cannot establish the diagnosis. Colour change, rapid growth, bleeding, crusting, pain, drainage, or a location close to the eyelid are reasons to have the bump identified rather than treating it as routine milia.

Do not cut, lance, or force the bump with fingers or a tool. Milia have no open pore through which the contents can simply be squeezed, and attempts may cause infection, bruising, pigment change, or scarring. Use gentle cleansing and avoid repeatedly rubbing or picking the site.

A dermatologist can confirm what the bump is and discuss observation or controlled removal. Topical retinoids may be considered for some patterns of milia or blocked pores, but they can irritate skin and should be kept away from the eye unless specifically directed. A large solitary lesion may need a different procedure.

Arrange a non-urgent visit when the diagnosis is uncertain, the bump is enlarging, it repeatedly returns after removal, or it bothers you cosmetically. Seek earlier care for increasing pain, warmth, redness, swelling, or pus, which suggests inflammation or infection rather than uncomplicated milia.

A lesion that bleeds without being picked, forms an ulcer, affects vision, or produces significant eyelid swelling deserves prompt assessment. Do not attempt treatment on the eyelid margin yourself.

Ask what features support milia rather than a cyst or whitehead, whether removal is necessary, and which method suits the location. Discuss irritation, pigment change, scarring, aftercare, and what to do if the bump recurs.
