Bronchodilators for asthma: relief, technique and safety

Some bronchodilators act quickly and are used as relievers; others last longer and may form part of regular treatment. They do not all treat the airway inflammation that drives asthma, so use the exact medicine and schedule in your prescription or asthma plan.

Asthma can present with wheezing, cough, chest tightness and shortness of breath because inflamed airways become sensitive and tighten. A quick-acting bronchodilator can ease this muscle tightening. The response may help a clinician assess asthma, but relief after an inhaler is not enough by itself to identify the cause of breathing symptoms.

A long-acting bronchodilator is generally prescribed within a planned maintenance regimen rather than used as an improvised rescue medicine. The device may contain one medicine or a combination, so colour and shape are unreliable guides. Check the label and know which inhaler is for sudden symptoms.

Ask a clinician or pharmacist to watch your technique with your own device. The correct sequence differs between inhalers: some require a slow, steady breath, while dry-powder devices usually need a quicker, deeper inhalation. A spacer can help with certain pressurised inhalers when it has been recommended.

Keep the reliever accessible, monitor the dose counter if the device has one, and replace an empty or expired inhaler. Record how often symptoms interrupt sleep or activity and how often you need relief. Increasing reliance on a bronchodilator deserves a prompt treatment review even when each dose still helps.

Call emergency services if breathlessness is severe, speaking becomes difficult, lips look blue or grey, you become confused or exhausted, or your prescribed reliever is not helping as directed in your asthma plan. Sit upright if comfortable and follow the emergency instructions in that plan while help is arranged.

Confirm the name and purpose of every inhaler, the dose prescribed, the correct device technique and what to do if symptoms return before another dose is due. Mention tremor, a racing heartbeat or other effects after use, as the clinician can check the dose, technique and whether another medicine is more suitable.
